Camp Enrollment Packet
This must be completed for EVERY camper regardless of method of registration
Camp Cancellation Form
Please submit as soon as you know you will need to cancel a camp week.
Camp Transfer Form
Use this form if you need to change a camp week for a different week.
Medication Form-Coming Soon
Form should only be completed if your child will be administered medicine at the YMCA. This includes inhalers, epi-pens and any other rescue medications that may be needed in the event of an emergency.
Authorized Pick Up Change Form
Use this form to add or delete authorized pick ups for your child.
